Design a treatment plan using behavioral
or insight therapy to address the problem/goal you have been assessing
throughout the semester.  Your paper will
include the following elements and must
be formatted using APA guidelines with the following headings/subheadings:
SELF ASSESSMENT: (20 points)
Conduct a self-assessment in the
following areas related to a current issue you are experiencing.  A self-assessment is designed to look at the
“big picture” and highlight contributing factors (successes and
challenges).   List the following:
1) Current symptoms and behaviors
·       Ex: Depressed mood, anxiety, change
in appetite, sleep disturbance, etc.
2) History of the problem
·       When did the problem begin?
·       What is the
intensity/frequency/duration of the problem?
·       What, if any, attempts have been
made to solve the problem?
3) Impairments in life functioning
·       Ex: Issues with home, school, work,
relationships
4) Family background
·       Socioeconomic level
·       Emotional/medical history
·       Family relationships
IDENTIFY GOALS for TREATMENT (10)
Once you have completed
the initial assessment, you will want to consider what interventions and goals
you want to create for treatment.
5) What is one goal you have for
therapy? What would you like to be different? What steps can you take to make
this happen?
6) Make concrete goals for treatment (list SMART goals).
Goals for treatment are what drive the therapy. The goals
also comprise a significant component of the treatment plan. Use the SMART goals approach (included are examples using “insomnia” or “depression”):
·       Specific – Be as straightforward as possible, such as reducing the severity of
depression or reducing nights with insomnia.
·      
Measurable – How will you know when you have achieved your goal?
Make sure it’s quantifiable, such as reducing depression from 9/10 severity to
6/10. Another option would be to minimize insomnia from three to one night per week.
·       Achievable – Make sure the goals are attainable and not too high. For example,
reducing insomnia from seven to zero nights per week might be a challenging
goal to achieve quickly. Consider changing it to four nights per week. Then,
once you reach four, you can get a new goal of zero.
·       Realistic and Resourced – Is this achievable with the resources you have? Are
there any other resources you need to help you achieve your goal before you can?
How can you access these resources?
·       Time-limited – Set a time limit for each goal, such as three or six months.
A fully formed goal might be: I will reduce insomnia from three nights per
week to eight nights per week in the next
three months.
DESIGN A TREATMENT PLAN (15 points)
7)       a. Design a
treatment plan for your
goal using a theory or approach from the text (behavioral, humanistic, etc.).  The plan must include at least three
statements with pro-active goals to achieve the desired result.  BE SPECIFIC – think about the SMART GOAL –
how will you get there?  
b.  Briefly describe why you chose this approach and cite the theorist most aligned
with your strategy.  (10
points)
