There are many agencies working together to work toward preventing errors in healthcare, yet they continue to occur frequently within our healthcare system. In this paper, you will describe a critical incident. This can be an actual error, a near miss event/potential error or an event that caused harm to a patient. Search the internet for patient stories related to this topic. Use what happened in that patient story to guide you through the rest of the paper. For example, you could find a story on a patient that had a wrong medication given to them. There could be a variety of reasons why that event occurred, such as the label was wrong, the medication was not scanned, it was ordered incorrectly, etc. Based on that event, find related scholarly articles and answer the following questions:
Describe the event from the patient’s perspective
What is patient harm? Include statistics that can be general or specific to your event. Was this event a near miss event/potential error, or an actual error that reached the patient?
Use the Model for Improvement (from IHI module QI 102) to complete a gap analysis of the event.
What data would you review? Include graphs if available.
Describe any barriers or ethical considerations that were encountered in the patient safety event.
How could this be prevented from happening to other patients?
What role does nursing informatics play in patient safety? How could informatics have helped to prevent the scenario from occurring?

Please use at least two scholarly references (other than the website you are getting your case study from and the informatics book) to back up your claims.
