Course Scenario: You are the new Director of Community Outreach Services at the local hospital in your community. Your duties and responsibilities include the following:
Assess the health needs of community members.
Provide recommendations and suggestions for new community service programs to hospital administration.
Develop a resource directory of local services, programs, agencies, and providers.
Develop comprehensive health service programs focused on prevention and education to address community health issues.
Implement and coordinate health service delivery and evaluate outcomes of health services program introduced to the community.
Promote and develop a broad range of wellness and outreach education services, including training, consultation, and direct programming.
Having just relocated to the area, your first task is to familiarize yourself with the community’s demographics and health statistics.

Part One
Complete Part One of the Data Collection Worksheet Template.
The data you enter on the worksheet should be based on a city or county, plus U.S. data. (International students may choose a region of their country and compare this data to their country’s national data.)
Before completing Part Two, carefully review the data and consider similarities and differences between the local and national data.
Part Two
Complete Part Two of the template in a 2- to 3-page paper, to include the following:
Identify similarities and differences between the local and national population health data.
Based on the data, identify at least two issues (health and/or demographic) that you view as “of concern” for your selected location and explain why.
Note: Justifications using personal beliefs, experiences, and/or anecdotes are not appropriate for this Assignment. Be sure to support your assertions with references based on specifics and data.
Explain why you, as the Director of Community Outreach Services for your health facility, would wish to address these issues for the community.
Identify other information that was not listed on your data collection table that may be useful in this type of analysis.
