Concept Map Assignment Instructions The purpose of this assignment is to help you strengthen a weakness and practice using clinical judgment in preparation for the NCLEX. Identify a concept from your “Weak Performance Areas” from your HESI dashboard. The selected concept* will be the focus of your assignment. Use the template provided as an aid for organizing the critical thinking and clinical judgement process for your concept and client information. *Please note that you may only use a concept once for each course.
1. Selecting a Concept a. “Weak Performance Areas”. * select a concept from the ‘Concepts for Nursing Practice eBook’ that can be found in the ‘Course Resources’ section of this course. 2. Client Selection a. View the parameters before choosing a client scenario. i. Step One: Choosing a Case 1. Select a client case that aligns with your chosen concept. 2. You can use a current patient, a past encounter, or a simulated experience if needed. ii. Step Two: Concept Focus 1. Develop your concept map around your chosen concept, even if it’s not the client’s primary concern. 2. For instance, if your concept is ‘Infection’, focus on cues and interventions related to ‘infection’ while considering the client’s other conditions. iii. Step Three: Multiple Co-morbidities 1. Understand that your patient may have multiple co-existing conditions. 2. Use the ‘Concepts for Nursing Practice eBook’ to explore related concepts while focusing primarily on your chosen concept for the concept map. 3. Building the Concept Map a. Use the nursing process and clinical judgment chart to organize client data before mapping concepts. b. View Nursing Process & Clinical Judgment Chart. After completing the nursing process template, utilize this information to help you build the concept map assignment. The arrows in the map show you the order in which the map is intended to be completed. All boxes should be completed related to the concept you chose. 1. Recognize cues. What matter the most? What client findings are significant, most important, and of immediate concern to the nurse? 2. What is the pathophysiology and disease process affecting this client? 3. Analyze the cues. How are the cues related? How do they link to the client’s conditions/problems? 4. Prioritize hypotheses. Rank the client’s conditions/problems in order of severity and complexity. What is most important? 5. Generate solutions. What are the desired outcomes for this client rated to your identified concept? Identify SMART goals for this client. What interventions support achieving the desired outcomes? 6. Take action. Implement your identified interventions. How will you accomplish these interventions? 7. Evaluate the outcomes. How will you determine the effectiveness of your interventions? 4. Completing the Assignment a. Utilize references in APA-style format. i.
