Health Promotion Program Proposal
In a 10-page proposal, please include the following information.
Title page including the project title, your name and title as the primary contact person, course, and section number

Introduction
Health status topic or issue impacting the county or state chosen (HIV in Haiti ages 15 and up)
Incidence and prevalence of the issue (use relevant data sources such as CDC, WHO, etc)
Statistics to support (use relevant data sources such as CDC, WHO, etc)
Community Description (1-2 paragraphs)
Describe the target audience(s) for the project (e.g., health practitioners, policy-makers, underserved population).
List the geographic location(s) and sites/settings (e.g., hospital, community-based center, school, work setting) where the project activities will take place.
Needs Assessment (skip elements should be in the intro)
Socio-ecological factors
Provide a description of the primary socio-ecological factors related to the health issue (e.g.. income, lack of health insurance, educational level).
Stakeholders
Identify and describe strategies used to identify your program stakeholders and build coalitions and partnerships.
Identify and describe types of resources and community stakeholders. Indicate the name of the partners you will work with during the project. Describe the role and contribution of each partner.
Mission, goals, and objectives
Identify the program mission.
Describe at least one program goal.
Describe at least two objectives.
Format

Theoretical framework
Identify and describe the theory (e.g., Health Belief Model) used to guide development of the interventions and provide a rationale for your selection. Do not use the planning model.
Intervention strategy
Propose an appropriate population-based program intervention strategy based on your theoretical framework that targets one of the socio-ecological factors associated with this health condition and target population.
Types of health promotion intervention strategies include health communication; health education; health policy; environmental change; health-related community service; or health advocacy and community mobilization.
Activities
Identify at least two specific program activities and explain how they would be used in the program.
Explain how each activity is related to your theoretical framework.
Logic Model
Create a Logic Model for the program.
Conclusion
Apply leadership and management principles in your take home message.
Describe implications for social change.
