Background:
Mr.
James Anderson is a 78-year-old retired African American male with a primary
diagnosis of Urinary Incontinence and Dehydration with intermittent
exacerbations. He also experiences poor sleep quality. Mr. Anderson has a past
medical history of benign prostatic hyperplasia (BPH), hypertension, and mild
cognitive impairment. He recently had a urinary tract infection (UTI) and was
hospitalized for dehydration secondary to poor fluid intake. His current
problems include urinary urgency and frequency, occasional urinary leakage,
fatigue, confusion, and difficulty falling asleep.

Tamsulosin
0.4mg PO qd for BPH, Lisinopril 10mg PO qd for hypertension, Amlodipine 5mg PO
qd for hypertension, Oxybutynin 5mg PO bid for urinary urgency, Melatonin 3mg
PO qhs for sleep.
Social
History:
Widowed
for 5 years with two adult children who live out of state. Mr. Anderson lives
alone in a senior apartment complex. He used to be active in his local
community center but has withdrawn due to his health issues. He enjoys
gardening but has difficulty managing it due to his urinary symptoms and
fatigue. Mr. Anderson relies on a caregiver who visits him twice a week for
assistance with activities of daily living.

health nurse visits three times per week for fluid intake monitoring, bladder
training, and assessment of urinary symptoms. Urology follow-up for evaluation
of urinary incontinence and BPH management. Fluid restriction education and
dietary consultation for managing dehydration. Cognitive assessment and
referral to a neurologist for further evaluation of cognitive impairment. Sleep
hygiene education and referral to a sleep specialist for evaluation of sleep
disturbances.
