Below are Clinical Scenarios. You will need to write a complete and accurate prescription for each scenario. This should look like a prescription a patient would receive from a provider. The prescription should include: patient name, DOB, prescriber name, signature, date, drug name, dose of each unit, dose prescribed, route prescribed, frequency, quantity prescribed, type of unit prescribed. If this is a pediatric patient, state the weight that your calculation is based on. If there is information missing in the scenario, you can decide what information you want to add. This should look like a prescription. Here is a template of a prescription, but it does not have to be exactly like this.

The prescriptions need to be submitted under the Submissions section of the class site. (save as Last name, first initial, RxNSG 488). Be sure to cite your reference used for drug dosage, frequency and duration (ex. Epocrates, Uptodate).

Clinical Case #1

J.M. is a 45 year old male who comes to your clinic for ear pain. On exam you find otitis media. He has a history of penicillin allergy, but has taken Cephalosporins in the past without reaction. You decide to prescribe Omnicef (Cefdinir) for his ear infection.

L.B is a 4 year old female who woke up last night with fever of 102 and crying and telling her mom her left ear hurts. You see her in clinic and diagnose her with otitis media. She does not have any allergies, but does have a history of a few ear infections per year and she is in daycare. The last ear infection she had was about 4 months ago. Patient’s weight is 40 pounds. Mom states she has never had a problem with Amoxicillin, she was given this at the last visit and did fine. You decide to give her Amoxicillin.

R.L. is a 28 year old female with a history of atopic dermatitis. She has had a recent flair with itchy flaky skin that is getting worse on her hands. She has tried otc hydrocortisone cream with minimal relief. She has required prescriptions before for this problem, but not for many years. You choose to give her triamcinolone ointment for the diagnosis of atopic dermatitis.

Case #4

M.B. is a 120 pound male age 17. He is in the office for asthma exacerbation with URI. You determine he needs refills on his albuterol inhaler.

J.K. is a 50 year old male with newly diagnosed Hypertension. He was placed on HCTZ 12.5 mg about 2 months ago for multiple BP readings around 150/ 90. He comes today and has BP at home running 168/96 on average the past few months. You choose to change him to Lisinopril/HCTZ daily because of his ongoing BP reading and a reading of 160/94 while in the office.
