1. Epworth Sleep Scale
The Epworth sleepiness scale (ESS) is a short self-assessment to identify how likely a client is to fall asleep during the daytime.
2. Mental Status Assessment of Older Adults: The Mini-Cog
This tool can be used quickly to detect cognitive impairment.
3. Geriatric Depression Scale
The Geriatric Depression Scale (GDS) is a patient-reported outcome measure to screen for depressive symptoms among older adults.
4. Beers Assessment of Inappropriate Drugs
The criteria’s goal is to reduce the number of potentially inappropriate medications (PIMs) that older adults are exposed to. PIMs are drugs that pose a risk of causing adverse events.
Complete assessments on an older adult at least 75 years of age.
