Assignment: HFACS Project (Applying the Human Factors Analysis and Classification System (HFACS) framework as described in Shappell & Wiegmann, 2000 from the assigned readings)
1) Do a search on the Internet to find one of the videos in the Aviation_Accident_Videos.pdf or another aviation accident of your choosing. View the video, paying close attention to the information presented about the event and particularly the findings from the formal investigation.
Note: It may be helpful to find detailed information for a select number of these aviation accidents at Lessons Learned From Civil Aviation Accidents (FAA) This website features a library based on type of aircraft (small airplane, transport airplane, & rotorcraft) with descriptions of some of the major aviation accidents and their related lessons learned. Three different “perspectives” (Airplane Life Cycle, Accident Threat Categories, and Accident Common Themes) are used to arrange the accidents in this library and illustrate the complex interrelationship of accident causes. Each accident also contains at least one high-level lesson related to a threat element and at least one lesson related to a theme element.
2) Apply the Human Factors Analysis and Classification System (HFACS) framework (as described in Shappell & Wiegmann, 2000) to classify the possible contributing factors to your selected accident in terms of the following areas:
- Unsafe Acts
- Preconditions for Unsafe Acts
- Unsafe Supervision
- Organizational Influences
3) Prepare a project summary that begins with a brief description (somewhere around half a page) of the accident, summarizing in your words the basic details of what occurred. Then provide a detailed summary of your HFACS analysis.
Assignment details:
- Prepare the HFACS project summary in Microsoft Word® and limit it to no more than four single-spaced pages (exclusive of references) in 12-point font with one-inch page margins all around.
- Adhere to APA (2020) style guidelines in citing and referencing your sources.
