Students will work individually to draw up a case conceptualization summary that describes a client in need of service or intervention. Based on the case conceptualization, students will utilize the information included in the case conceptualization to complete a clinical interview to gather additional information from this client.
You will be asked to complete a clinical interview with an identified client. You will utilize one of the sample clinical interviews and psycho-social intake(s) under the subfolder of “Psychosocial Intake Templates and Examples” found on Canvas. You will use this clinical interview in your subsequent treatment plan, clinical notes, and assessment assignments within the class.
Please make sure to use a fictional client/former client (with de-identified information) and their information in the clinical interview that can be used for your other assignments.

Based on the information you obtained in your clinical interview/case conceptualization assignment, you will identify what assessment methods and measures should be utilized in the case with the client.

Each student will employ various assessment methods and measures based on their clients’ specific problems. To further aid your understanding of this assignment, I have uploaded two example papers. These example papers demonstrate how different students have utilized various assessment methods and measures to gather and document information about their clients’ psychiatric conditions and symptoms, functioning, behavior, personal history, values, preferences, goals, and other pertinent details.

Remember to choose an assessment that will assist you in determining an appropriate diagnosis for the client and will assist you in creating an effective treatment plan. Such assessment methods and measures can be found in Module 3 or Module 4 for a wide range of client ages and presenting concerns. 
These assessments include BASC-2, PHQ, GAD, Beck Inventory for Depression, Ages and Stages, trauma, mood scales, other anxiety assessment, etc.  To find the most appropriate assessment methods and measures for clients’ issues, students are encouraged to thoroughly review all the materials.
Assignment 3: Treatment Plan (6.5 pages) (2 resources)
Please thoroughly review the instructions and utilize the following resources to complete this assignment.
The Treatment Plan Format outlines the essential contents that should be included in this assignment. It is recommended to utilize three subheadings:
1) Brief Summary of Intake and Assessment,
2) Diagnosis, and
3) Treatment Plan
Please note that specific information will vary among students based on their individual case conceptualizations, the assessment tools used, and the treatment plans designed to address their clients’ presenting problems.

Go to Module 6: Treatment Planning to review Treatment Plan Structure examples and materials 
Remember to add an APA-appropriate cover page (do not use the generic one on the Treatment Plan Structure example).
Please follow this Treatment Plan Structure example as guidance for ALL the information that must go in each section of your individual paper. 
You may insert the CBT Case Conceptualization Table as in the example or organize the table sections with headings. 
Quick tip: I will be paying closer attention to your completed Case Conceptualization table or section to assess your ability to answer the question: “What does CBT theory say about why this is happening to my client now?” and connect it to the treatment goals and objectives.
With the Treatment Plan Structure sample as a guide, this assignment should be much easier to complete. Simply add your client’s actual or hypothetical information in each section.

Assignment 4: Clinical Note Taking (1.5 pages)
This will be an assignment in which you will provide a clinical note of actual client session with your client. You will utilize your created treatment plan as your guide in identifying what topics should be covered in the therapy sessions, and what goals you should address in that session.
****Remember that you should always explain confidentiality, recap what was learned in a previous session and gather homework completed by the client, review any data collected and progress, discuss what you are working on in session for that day, conduct the session, summarize the session events and your findings, and then prepare the client for the next session.
