Community Intervention Project Plan

A community intervention project plan engages the student in a process of conceptualisation where students must analyse the issue to better understand the extent of the issue to conceptualise and recommend the most suitable intervention plan. The project plan will outline all the steps and processes to be followed, as well as all the stakeholders/collaborators involved and activities to be engaged in.
The student’s ability to conceptualise a plan to be implemented in response to the identified community-related issue will be assessed.
Instructions:
Choose a social, economic, or psychological issue that affects your community or another community. Imagine that you have been tasked with creating a program to address the issue in that community. Your task is to conceptualize and develop a community intervention project plan to combat the problem you have identified.
Your write-up should contain the following:
A detailed description of the issue you have identified and an overview of the community where the issue occurs. Include a description of how the issue is perpetuated within the community context.
Outline the specific steps for conducting a needs assessment to better understand the needs related to the issue you have identified.
Analyze the scope of the problem using an applied ecological systems approach.
Create a detailed intervention plan that outlines how you will address the identified issue and alleviate the problem.
Explain how you will engage and empower the community to participate in the intervention plan.
Identify key indicators to evaluate the potential strengths and challenges of the program.
Discuss how you will measure the success of the intervention plan.
