I enjoyed reading the information presented in this week’s module and learning about just culture. One of the articles I chose for this discussion is “Fostering A Just Culture In Healthcare Organizations: Experiences In Practice”. Recognizing just culture as a multidimensional concept, this study highlights requirements and challenges for implementation in practice. Three requirements were identified for fostering just culture in healthcare organizations: “Open communication, room for emotions and involvement of management” (van Baarle et al., 2022). Given their complexity, the authors counter this discussion by mentioning specific challenges within each attribute. For instance, when considering involvement of management, how can managers support their team while maintaining a distance to drive performance? (van Baarle et al., 2022). The study concludes with recommendations and solutions for overcoming potential dilemmas in just culture. One solution is also a recurring theme presented throughout the paper: The “importance of postponing judgement and a listening attitude” (van Baarle et al., 2022). Another solution of equal weight is cultivating an environment supportive of reflecting on the situation itself, as well as the personal emotions evoked by an incident.
The second article I chose for this discussion is titled “Just Culture In Healthcare: An Integrative Review”. This study attempts to expand the current body of evidence of just culture in healthcare and demonstrate its relevance in promoting patient safety. Through a comprehensive literature review, Barkell & Snyder (2021) explore the role of error management, balance, leadership and staff, and systems leadership for change, along with their implications in just culture. Based on the literature analysis, the authors found a need to increase error/near miss reporting and improve opportunities for learning (Barkell & Snyder, 2021). Additionally, they emphasize the importance of future studies highlighting the impact of just culture on patient safety and outcomes, as opposed to a focus on safety culture alone. These suggestions for implementing just culture show promise for improving safety in healthcare, in addition to providing a platform for new evidence and quality improvement projects.
When linking the just culture articles back to the five critical appreciative leadership skills, the two attributes that stick out the most are catalysis and positivity. When considering the message of the first article “postponing judgement and maintaining a listening attitude”, the appreciative leader must exude positivity in their role, for their teams, and the broader ecosystem they serve (van Baarle et al., 2022). When building a just culture, modeling positivity during group discussions is key especially when the topic relates to errors, which can trigger an array of individual emotions. Also, in this case, the appreciative leader is a central catalyst that inspires the group to shift from blaming and judging an individual to understanding and resolving errors on a systems level. This transitions into a group synergy that empowers their ability to work together toward a common interest, such as patient care. Relational integrity is activated by the appreciative leader when team members perform for their own personal benefit, and for a greater good of others. For instance, linking just culture to improvements in patient safety and outcomes is a result of relational integrity. Finally, when presented with errors, near misses or common dilemmas, appreciative leaders readily identify solutions that restores the group’s functions. In just culture the leader would encourage open, honest discussion, active listening, and identifying processes aimed at preventing future mishaps.
