C) Explanation of Content/Theme of
Paper Using the ‘transtheoretical’ (behavior
change) model, select a health-related behavior to change over a span of six
months. The behavior should be exact. For example, to improve ‘physical
wellness’, select a specific detail (body fat percentage, cardiovascular health,
or specific body part, etc.) to change. Use the six steps of the model as
headers for each section: Pre-contemplation, Contemplation, Preparation,
Action, Maintenance, Termination. A minimum of TWO (2) sources must be used for research. DO NOT use sources before 2015.
All sources will be listed on a separate page following the end of the paper.
1) Pre-contemplation- Specifically
explain how the problem is negative in your life including why you cannot
currently fix the problem. What is holding you back? Identify & detail the
personal defining moment causing you to finally decide to fix the problem. This
should be at least two substantive paragraphs.
2) Contemplation- Analyze
& summarize how your life will change during execution of the plan. What will you sacrifice? How will you adjust
your schedule to create time for the modification plan? What are possible
problems & how will you address those? This should be approximately two to
three substantive paragraphs.
3) Preparation- Include all research, but not simply listing random
stats. Explain personal interests guiding your research & decisions about
the direction of the plan. What did you learn about the issue you want to
change? What mistaken beliefs (if any)
did you have before research? Based on research, what did you learn specifically
that might work & strategies to avoid? Draw conclusions about how the
research will guide your attitude/mindset when preparing the plan. Study
varying demographics in your world & beyond, including global perspectives.
If perchance the percentage of people your age having your issue is lower in a
different part of the world, research ‘why?’ Incorporate it into your personal
plan if appropriate. This should be at least four to five substantive
paragraphs. It might be the longest section of the paper. 4) Action- Describe every small detail of the
plan from the day you begin to the day you end. Include backup plans if you
encounter a problem along the way. How will you track progress? Who will assist
you with motivation? How will you alter the plan if your life suddenly changes
instead of quitting? This should be at least three to four substantive
paragraphs. 5) Maintenance-
Describe what happens after achieving your goal. What behaviors will ensure you
do not backtrack/relapse & return to the bad behavior? This should be at
least one (maybe two) substantive paragraph(s).
6) Termination- Describe in detail the type of person
you will be following plan completion. This should be at least one (maybe two)
substantive paragraph(s).
