Description of Proposed Research/Project Description (including goal(s) and objectives, methods/approach/activities)
This section is the heart of the proposal and is the primary concern of the technical grant reviewers. A description of the proposed project will be provided.
Now that a need/problem has been identified, funding will be requested to develop and implement the project to address the need.
Describe why you/your organization are best suited to conduct what you propose to do? Is the proposed project an extension of successful, innovative work or a pilot project already completed? Be realistic about what can be accomplished during the funding period. Not applicable to all projects.
• What is the project research questions?
• Be explicit about any assumptions or hypotheses the research method rests upon
• Target population/Clientele
o Include specific information on the population or clientele that your project is focused on.
o Exactly who are the clientele? Who is included/excluded in the clientele group? In what ways have you already had contact with the clientele group?
o Can you show that you have the support of the clientele group to move ahead with the project?
o In what ways (if any) have members of the clientele group been involved in the preparation of the proposal?
o What other agencies are involved with this clientele group (and have these other agencies been included in your proposed project)?
• What is the goal(s) of your project? Be realistic about what can be accomplished (SMART Objectives – Specific, Measurable, Achievable, Realistic, and Timely) and who will do them?
• Be certain that the connection between the research objectives and the research method is evident.
• The objectives will form the basis for the activities of your project and will also serve as the basis for the evaluation of your project.
• Present measurable objectives for your project. If you are dealing with “things” it is easier for them to be measured than if you are dealing with abstract ideas. Your proposal is easier for a prospective funding organization to understand (and the outcomes are much clearer) if you describe your objectives in measurable ways.
• What will your project’s outcomes be? Discuss the specific outcomes to be achieved? What will change?
• How are you going to achieve those outcomes? What methods will you use?
• Be as detailed as possible about the schedule of the proposed work.
• Describe the specific activities involved.
• The prospective funding agency will be looking at your methods to see what it is that you are proposing that will be new, unique or innovative. Make sure you clearly present the innovative aspects of your idea.
• Present a timeline of activities. Tables and charts work best here. They crystallize data, break up pages of narrative, and convey extensive information well in a limited space.
• Discuss why you chose to address the issue in the manner being proposed. Are there other methods/approaches to address the identified need? If so, why are they not appropriate for the situation? Be clear about the focus of the research selected.
• Delineating the impact is important because funders want to see that you have clearly established the realistic benefits of your work along with how you plan to verify and assess your achievements.
Grant Proposal: The integration of service and communication in the Health sector.
The medical community is a sector that quickly embraces significant data advancement, but implementation tends to be rough. Non-associated databases combine patient information from various sources by providing actionable metrics (Berg, 2014). The aim of the paper is to discuss on problem of integration of service and communication in the health sector and propose a recommendation.
During the old times, healthcare providers relied on relational records to access and store patient information. Nevertheless, the relational databases failed because they could not manage unstructured data, including transcripts and clinical notes, effectively. Only a small number of healthcare practitioners had the technical knowledge on data transcription from personal to non-relational databases using traditional electronic medical records. The majority of the organizations effectively implementing the non-relational medical records are large and have vast financial resources. The benefit of the non-relational databases is that they can fully exploit the received patient information regardless of their format. The technology assists healthcare providers in identifying traits that were lost after they were recorded or until they have been reviewed individually. This makes the process expensive, and therefore only the well-up institutions can afford the technology.
Concerning this, the process of encouraging patients to share their medical information is becoming critical, with some patients becoming reluctant to share their information with caregivers, including the drug manufacturers. This is making even the health facilities that have implemented the non-relational databases continue to worry if they will be able to secure their patient’s information as promised. Due to the absence of adequate information from the clients, the health sector is wasting valuable resources on research and development, which is passed on to consumers as tax and increase in healthcare costs burdens (Kinder, & Burgoyne, 2013).
Therefore, my grant proposal for the problem is for health sector to come up with strategies to educate patients on the necessity of information sharing with them by establishing more personalized and customizable information sharing plans.

The theory of my assignment is 1. Diffusion of Innovation (DOI) theory that originates from communication to illustrate the way the idea or product gain momentum and spread to other people easily. The integration of service and communication in the health sector brings together different parts of health and social care to provide a relevant way of sharing information and avoid some of the communication confusion that arises in the facility (Elrod & Fortenberry, 2020). The integration of services and communication in the health sectors is beneficial because the services are made accessible easily and create a good understanding and use of information leading to overall good patient health. The theory of Diffusion of Innovation is important in this project because it suggests how the idea will spread to more people and be adapted by many.
2. The theory that I will use is the Diffusion of Innovation theory. It was developed by E.M. Rogers and it was used to study that the adaption of new ideas, products, or behavior does not happen at once but requires process and plan. As applied to my study, this theory holds that I would expect my independent variable will help in holding to the variable of health care to influence or explain the dependent variable the use of integration of service and communication (Kaminski, 2011) because the theory will help my project implementation because it gives guidance on the benefit of integrating service and communication in health sectors.
The goal:
The goal for my grant proposal is to increase accessibility to patient health and medical records by all health care providers in New York City.
SMART objectives:
70% of healthcare providers in New york city will attend HIPPA training online for an HOur by April 2022.
By March 2023 90% of health care providers in New York City will manage patient’s health and medical data, According to HIPPA regulations and guidelines.
By April 2023 80% of new York city healthcare providers will have access to patient’s health and medical records.
