Collaborate with your local health department to conduct research. Gather data (as identified below) on the aggregate or vulnerable population in your community.
Include and provide examples of:
economic conditions,
common genetic trends related to certain disease processes, and
trends in health-related behaviors or disease rates.

Interview a member of the aggregate or vulnerable population in your community that you have identified. Respectfully ask questions related to the impact of economic conditions, environment, family history, and other factors on health risks.
Step 4
Begin an APA-formatted paper to assess the aggregate or vulnerable population and community health issue you identified. In your introduction, provide a brief description of your community including the city name, state, and key features.
Briefly introduce the aggregate or vulnerable population that you identified in your assessment and research. Support the selection with evidence from your windshield survey and interview.
State the purpose of your paper. The purpose statement of the paper should relate to assessing this one aggregate or vulnerable population in your community.

In a section with the heading “Vulnerable Population Overview,” include key information and examples related to the identified vulnerable population:

Events or trends affecting this group (based on your interview and data collected from your local public health department)
Economic conditions, common genetic trends related to disease processes, and trends in health-related behaviors or disease rates
Strengths, risk factors, and/or barriers impacting this group’s health
Step 6
In a section with this heading, include key information related to “Community Resources”:
Health resources available in your community for this vulnerable group.
Other resources in your community available for this group.
Address these questions:
Are these resources adequate? If so, why? If not, discuss gaps in services that you identified.
Support your statements with information from your interview, windshield survey, and data from the public health department.
Step 7
Use the data you have gathered to draw conclusions and formulate a section in your paper with the heading “Community Health Problem Diagnosis” based on your assessment and the Healthy People 2030 objectives:
Identify and discuss one priority community health problem that a community health nurse could positively impact. The problem must relate to the aggregate or vulnerable population you identified, your interview, data collected from the local public health department, and your windshield survey.
Identify a theoretical framework to address the issue.
