What areas may need further assessment in order to clarify any possible misunderstanding? How would you go about prioritizing challenges observed? What tasks and professional case management roles would you engage in to serve the client / clients?).
Present an illustration of a possible Case Management Plan of Action (i.e., goals, objectives, plan for delivering services as well as evaluation of service effectiveness). See chapter 19 & 24 (Figure 24.4) of the Summers book . This is an opportunity for you to answer the:
Who – person / people providing services and facilitating the needed intervention(s). What knowledge and skills are essential? What goals and objectives will be established?
What – important modality / modalities (i.e., outreach, educational activities, counseling, etc.) that will be applied in order for the identified needs / challenges to be addressed while promoting culturally relevant / sensitive services. This is an opportunity to identify and analyze the interventions being considered.
Where and When are services provided? Are services accessible? (i.e., consider accessibility and availability based on frequency and duration of services and clients’ needs).
Why is the service important (i.e., if services are not provided what may be future implications)?
Strategies that will be used to assess service effectiveness as well as to strengthen professional use of self, increase awareness, and promote effectiveness? Ways of confronting possible biases / stereotypes / counter-transferences?
Reminder: Remember to be clear, organized, and comprehensive / concrete with your responses. Examples are often useful after making general statements. Examples allow the reader to assess the depth of your understanding and possible skills when assessing and working with a possible client.
