Because patients are unique with their knowledge, culture, self-care capabilities, resources, etc., it is important for the nurse practitioner to be able to assess and utilize different models of care based upon the presentation of patients. As a provider, one must be able to best way to evaluate what the patient knows and be precise with educating the patient in a time-conscious community. The purpose of this Discussion Board Assignment is an implementation/integration of models of care utilized in the clinical setting to determine best application to patient encounters.

In this assignment you will select one theory reviewed in Module 4 and develop a patient scenario and 4 well formed discussion questions for your peers to answer in a reply post next week. This module is 2 weeks long. Week 1 initial discussion is due by 6/27 at 11pm. Week 2 reply discussion is due by 7/6. ***Please only one reply per discussion post (to start). Once a post has a response, you may add an additional response if you would like, but please make sure each of you choose only one post for your initial reply.***. Keep reading below for an added benefit for completing more than one response. The first 4 parts will be included in week 1. Part 5 consists of the week 2 response.
Select one of the theories reviewed in Module 4 and follow the directions below. Please avoid use of direct quotes in this assignment. I want your summary, not the author’s summary when utilizing reference materials.
Part 1: Introduction: Discuss in general how a nurse practitioner incorporates nursing models and/or theory in the clinical setting and patient care. How does using a model to ask questions lead to a better understanding than just simply being random in the questions you ask?
Part 2: State which theory you have chosen for your discussion. Explain why you chose the theory. Was the decision based on the patient’s history and background, or in relation to the anticipated process of addressing the issue/health behavior you have chosen to address? Include any additional information or support for your decision. (Please use scholarly support to supplement your opinions)
Part 3: Develop and share a detailed Case Study style patient scenario in which a patient is struggling with a health behavior you wish to address (Not a medical diagnosis!). The patient scenario can be based off of a real patient or fictional to meet the purposes of this discussion. Only include relevant/pertinent demographics, patient history, cultural information, social history, etc., and not an entire patient history. [For example, if you are addressing the patient not getting mammograms, then you need history only related to this breast health issues or past education or past attempts to get this screening, etc. Some past history would be important to include and some would not.]
Part 4: Based upon your patient scenario and theory choice, develop 4 discussion questions to which a peer will respond (next week). When you are developing your questions, think about why you chose the theory you did, and why it was a good choice. How will you be able to determine if the theory has been correctly applied? How will you be able to measure if the patient did, or will, benefit from the education/intervention? (This is where reading the articles include in the module may be beneficial to you. Take a look at how the authors integrate the theory into their research and patient interactions).
Part 5: By 7/6, please choose ONE peer’s discussion questions to answer. Please provide thorough and detailed responses to each of the 4 questions using scholarly support for your responses. Responses should consist of more than just 2 or 3 sentences. Once everyone’s posts have a reply, please feel free to respond to an additional post. (**Hint…you may want to consider doing this… 5 additional bonus points will be given for doing so – as long as the reply is as thorough as your initial response**)
Please use at least 2 scholarly sources (must be 5 yrs or less), including journal articles, to support your discussion. One of your journal articles must be related to the models and discussion. Do not use non-healthcare related sources or sources that are not intended for health care providers. Do not use web pages that are for the lay public or not peer-reviewed. (Example, using a personal or private developed web page for a theorist is not appropriate when better information is derived from your textbook/readings).
