Directions
Create a concept map presentation that shows connections between social determinants of health and nursing practice. Be sure to keep text to a minimum on the slides and provide depth and detail in your speaker notes. Refer to the Supporting Materials section for additional information. An optional template is provided in the What to Submit section. Use at least two current (within the last five years) references to support your work.
Specifically, you must address the following rubric criteria:
Five Domains: List the five domains of the social determinants of health.
Individual Health: Explain how each domain impacts individual health. Use a scholarly, current (within the last five years) reference to support your explanation.
Perceptions and Beliefs: Identify your perceptions and beliefs related to social determinants of health.
Culture: Identify your perceptions and beliefs related to culture.
Population or Community: Describe how social determinants impact the health of a population or community both positively and negatively. Use a scholarly, current (within the last five years) reference to support your descriiption.
Resources: List three resources to provide to the individual to increase their knowledge or awareness of SDOH. Note: These resources are different than the references you’ve used to support your work on this concept map.
What to Submit
Submit your completed Module One Concept Map Template or a completed PowerPoint presentation of your own design. Your submission should be a one-slide PowerPoint presentation with separate title and reference slides that are not part of the slide count. Include detailed speaker notes with your presentation. Follow APA style standards for formatting and citing your sources.
