Section one (3-4 pages) should include a discussion of the problem and goal specification. (8 points)
A brief background statement including the presenting problem, family information, psychosocial history, and other information relevant in framing the case.
A brief discussion of the assessment process based on CBT, including specification of target problem(s), plan for measuring the problem(s), and at least 3 goal(s) for the problem(s), and at least 3 objectives to meet the goals.
Section two (5-7 pages) should include a discussion of the process and the outcome or anticipated outcome of the intervention. (12 points)
Discuss the treatment plan selected to reach the goal(s). The plan should explicitly outline how the client and worker will address the problem (as specified in the assessment process) to reach the desired goal (as specified in the assessment process). This should include the 3 goals and the associated objectives (listed in the above assessment section), along with specific strategies on how to obtain these goals and objectives.
Based on CBT please discuss: what the intervention process will consist of specifically the structure early, middle, and ending phase sessions; the nature of the client and social worker relationship; any special issues of consideration while engaging in the CBT intervention process. Please include a discussion of the proposed time frame for this intervention process.
Please give a brief discussion of ethnic/cultural (including social class) factors and how these would be addressed during the intervention process. Please include a discussion of possible barriers/ issues that may come up during the intervention process (such as lack of engagement and participation (whether due to resistance or external barriers), transference and countertransference, agency conflicts, etc).
Include any anticipated/ possible collateral contacts (such as court; probation; school system; medical professionals, etc.) and purpose and relevance of contact in therapeutic process.
Based on the treatment plan and CBT approach please discuss anticipated outcomes and how they will be measured, as well as what termination with this client may look like.
Complete Conceptualization/Treatment Plan Form that will be provided. This will be added as an appendix to the paper.
