Phenomena of Interest (POI)
The approved POI is utilizing telehealth to deliver cognitive-behavioral therapy (CBT) to vulnerable populations such as caregivers of persons with mental health disorders. The POI is relevant to my current practice specialty as it provides knowledge and insight on the need for delivering telehealth CBT to the caregivers of mentally-ill patients in order to promote timely assessment, diagnosis and treatment for mental health issues and improve their wellbeing and positive mental health status and outcomes (Saimaldaher et al., 2020). Mental health problems affect quality of life and increase risk of developing mental health issues. Telehealth improves access, reduces costs, and increases flexibility of mental healthcare services (Park et al., 2021). The essence of my POI is to offer improved outcomes through more effective treatment for vulnerable populations such as caregivers of persons with mental health disorders.
Philosophical Influences
The preferred philosophical viewpoint is continental philosophy because continental
philosophy is concerned with the connection of an idea to the world around the idea and its
historical context (Note et al., 2021). Through continental philosophy, I will analyze the POI and assess how telehealth can help improve quality and accessibility of mental health services. The philosophical viewpoint will help make the connections between the mental health of patients and the mental health of their caregivers. I will use continental philosophy to assess how self and self-consciousness, desire and will contribute to mental health problems (Note et al., 2021). The selected philosophic viewpoint will ensure that I achieve the goals of my POI.
Ethical Framework
The ethical framework that will guide decision-making in my advanced practice role is dignity enhancing care framework. The framework explores vulnerability, care, and dignity that must be observed in an ethical approach to nursing (Mallari & Tariman, 2017). The framework proposes that healthcare professionals but respect human individuality and treat each individual as a unique human being (Ventura et al., 2021). The essence of nursing is to promote human dignity and therefore nurses should always promote respect for human dignity (Ellis, 2020). Social or economic status, personal attributes, and the nature of health problems should not restrict provision of safe and quality care. Based on the ethical framework, I will ensure that I develop interventions that promote human dignity for all patients.
The first ethical principle upheld in the chosen ethical framework is nonmaleficence. The
ethical principle requires the avoidance of harm (McDermott-Levy et al., 2018). The ethical principle supports my advanced nursing practice in addressing the ethical concern because it seeks to ensure that nurses formulate interventions that cause no harm to the patient. The selected ethical framework helps promote human dignity and this can only be actualized without causing harm to the patient. The second ethical principle upheld in the chosen ethical framework is beneficence. The principle of beneficence holds that each action undertaken by healthcare professionals should be done for the benefit of the patient (Grace & Uveges, 2022). Incorporating patients’ beliefs in the treatment plan helps ensure provision of holistic and patient-centered interventions.
Theoretical Influences
The grand nursing theory related to the POI is Sister Callista Roy’s adaptation model. The Roy adaptation model points out that the aim of nursing is to increase compliance and life expectancy (Kumar et al., 2022). According to the model, assessing patient in physiologic mode, self-concept mode, role function mode, and interdependence mode helps ensure provision of holistic care (Kumar et al., 2022). The middle-range theory that is related to the selected POI is Rozzano Locsin’s technological competency as caring in nursing. Utilization of telehealth involves use of technological resources and competencies (Krel et al., 2022). The theory represents the theoretical starting points for using technology and caring as a coordinated interaction in the nursing environment (Krel et al., 2022). The Roy adaptation model aligns with Locsins theory because it seeks to increase patient compliance to ensure positive patient outcomes (Callis, 2020). The use of technology such as telehealth as proposed by Locsins theory can also help increase compliance. The Roy adaptation model aligns with Locsins theory because it advocates for assessment of specific patients’ needs to ensure delivery of holistic care (Callis, 2020).
Complex adaptive systems (CAS) are a dynamic network of interactions made of different components (Paley, 2007). Under the complexity theory, CASs are viewed as open systems that interact with their environments (Olsson et al., 2020). The theory helps us understand the systems environments before understanding the systems (Olsson et al., 2020). The specific CAS within the POI is service delivery. Service delivery through telehealth maybe impacted by different systems and agents. CAS theory will be used to understand the effects on safety and quality of care after introducing telehealth in delivery of CBT. Each primary care practice will be viewed as a complex system with unique features. Utilizing telehealth to deliver CBT to vulnerable populations could have unpredictable effects and improve accessibility of care in some areas and increase health costs in others. The principle of flexibility will be adopted to ensure that different approaches are used to address issues that may arise in utilization of telehealth (Olsson et al., 2020).
