Set up of paper:
1) local Australian community geographical location (less than 50 words)- identify the name of LOCAL Australian community and briefly state the LOCAL Australian community geographical Location ie Murray Bridge..

a. Identify and analyse within the target group what are the MOST dominant individual or contributing factors xausing the health problem. Note-Individual/contributing factors are known health risks that directley contribute to the problem ie.. disease, illness or condition…. poor diet smoking, diabetes…
b. Determinates of health in LOCAL community that influence the target groups health problem.
Which is having the greatest influence on health and what are the root causes of the health problem and status…. (social, economic and environmental factors.)
5) (200 words) State what are the issues faced by the target group in relation to how they can access health care.
Apply Levesque et al (2013) conceptual framework of access to health care and state the factors that may be affecting the groups ability to reach out for support by also considering the 5 A’s in healthcare delivery.
6) State the partnership opportunities (200 words)
a. who the nurse could partner with on addressing this health problem and list at least 2 partners from WITHIN the health sector and 2 parnters from other sectors, these partners MUST be OUTSIDE of the health sector and sysytem.
b. briefly state the purpose/rationale and the partnership level of collaboratiing with each other or stakeholder. Partner can be teams, people or organisations
