The health and wellness of our community are vital components of a thriving and productive society, however, not all members of the community have the same health outcomes due to various social determinants of health like transportation, housing, and safe neighborhoods. For example, the lack of preventative screenings for chronic diseases like diabetes in lower-income areas has a direct correlation to advanced complications and a higher cost of care for diabetes. The sex, gender, ethnicity, social-economic status, and age of an individual or community can make them more susceptible to specific diseases or health problems that result in serious health disparities. For us as health professionals to design, implement, and evaluate successful community health education programs, we must learn more about the demographics, strengths, challenges, needs, and characteristics of the target community, while conducting an in-depth analysis of the disease by looking at preventative measures, interventions, and the resources available or missing to mitigate the impact on the individual or community. 
For this second written assignment, you will be conducting a comprehensive analysis of an assigned disease and the impact it disproportionally has on a specific community, along with identifying resources, programs, and interventions at the primary, secondary, and tertiary levels. For example, Latino and Black community members have higher rates of diabetes than their White and Asian counterparts. Why is that? What does the data reflect? Is it because of the lack of resources, health education, health facilities, genetics, or cultural norms? What type or level of interventions are available or missing that will improve their health outcomes? Are there culturally appropriate and relevant programs available? Gathering and analyzing all of the answers to these questions in the design phase of interventions and including members of the target population in all stages of the intervention will result in effective, culturally appropriate, sustainable, and impactful interventions.
