Section 1 – Identification and Assessment of Health Challenge: 10%
Each group will select a health challenge in consultation with the instructor before writing the first section of the health program plan. Section 1 will include tthe following parts, which should be clearly labeled in the paper:
Part A: Definition and discussion of the health challenge
Part B: Description of the population(s) affected by the health challenges
Part C: Description of the factors associated with or causing the health challenge
C1: Include factors under social ecological model levels of the individual (genetics), individual (behaviors), and interpersonal
C2: Include factors under social ecological model levels of community, environmental, and societal
Part D: Discussion of previous interventions that have addressed the health challenge and any evidence-based strategies/best practices recommended for intervention

The paper should be typed 12-point font, with 1-inch margins all around, double spaced, and 6-8 pages in length (excluding the cover page, reference list, and optional appendices)
Section 2 – Program Model, Activities, and Timeline: 10%
Section 2 will include the following parts, which should be clearly labeled in the paper:
Part A: Goals and objectives of the health program
A1: Process objectives
A2: Outcomes objectives
Part B: Description of intervention and program activities
Part C: Timeline chart (using Excel template) and narrative description for a timeline of the health program

Section 3 – Evaluation Plan and Budget: 5%
Section 3 will include the following parts, which should be clearly labeled in the paper:
Part A: Evaluation plan for the health program

A2: Outcomes evaluation
Part B: Program budget tables (modeled from the tables described in class) and justifications

So I am foucusing on the Bronx, New York. You have to focus on a specific neighborhood in the bronx. After that you have to see how is hiv/aids is being transmited wheater that be thorugh sexual intercourse or needles/drugs. Then you have to tailor the program to combat that. I attached the presentation slides to see how we were going to do the plan.
