(urgent care center, dental office, medical office, pediatric office, ambulatory center, etc.) What is the name of the facility? What products and/or procedures do you offer? How large is your facility, meaning how many departments make up the organization? (i.e. Administrative Department can consist of the front desk, scheduling department, and business/financial department. Clinical Departments consist of surgical, recovery, hygiene department, etc.) How many healthcare providers and staff members do you employ? Please explain the different posts/positions that make up the staff at your facility. (doctors, dentists, nurses, scheduling coordinators, treatment coordinators, hygienists, acupuncturists, receptionists, radiologists, insurance coordinators, etc). If you have multiple departments, are there managers for each department or are there levels of management? What kind of a leader/manager are you? What is your leadership style?
