A 19 year old male who is currently an undergraduate college student at Brown University presents to you, the on call nurse practitioner of the University infirmary, the night before an important organic chemistry exam. He states that approximately 4 days ago, he had attended an informational meeting for the campus Theater Club; he notes that while at this meeting, he encountered a girl who he describes had been “hacking” at her lungs and coughing up large amounts of whitish, thick sputum (mucus). He noted that not even 6 hours after the meeting, he began to experience some significant chest tightness, and the following morning, he began to cough up significant mucus. He also noted that around day 3 after the meeting, he experienced subjective chills, stating that he felt as if he would go from “hot to cold.”
He took his temperature prior to coming to you and notes that it was 99.3 degrees Fahrenheit. You take his vital signs, which indicate a mild level of tachycardia (increased heart rate) at 104 beats/minute, tachypnea (increased respiratory rate) at 22 breaths/minute, and a temperature of 100.3 degrees Fahrenheit. You auscultate his chest with a stethoscope and hear mild wheezing and crackles but, otherwise, no significant finding which would indicate serious pathology such as pneumonia. He otherwise appears to be very healthy.
He notes that he has had similar events happen in his hometown of Tacoma, Washington, for which he was given a “bubble gum” flavored medicine called “Amoxicillin,” an antibacterial antibiotic. He is eager to take this again if at all possible before his exam!
You decide to perform labs on the young man, and his labs are fairly normal (with the exception of a mildly elevated white blood cell count). Bacterial cultures and testing are negative for bacterial infections.
