Achievement of Goals & Objectives
Describe how you achieved your goals and objectives set for your project. Additionally, you will provide the deliverables for the goals and objectives. The idea for this section is to give a thorough description of what you did during your project to meet the goals and objectives and provide evidence that they were achieved. List each objective and provide a thorough description of what was done to meet the objective. For example, if one of your objectives was to interview staff at the facility regarding their satisfaction with their work environment, you would include the interview questions, a summary of all the responses to the questions, and your analysis of their responses.
EBP Project Solution
Describe the plan of action for addressing the problem you proposed and the rationale for the proposed solution. You should describe your plan in detail, including a timeline and strategy for implementation. For example, you may plan to create an operations plan, develop policies, a handbook, or a manual, design a conflict resolution plan, or conduct employee education sessions. If you plan to revise a policy or procedure, include the original and the new policy in addition to discussing what you did to change the policy or procedure.
Evaluation Plan
Describe your plan to evaluate the outcomes of your plan. How and what will you measure to determine if your solution worked? If possible, present any real results you have from implementing your plan. If real results are not attainable, provide a thorough description of how you will evaluate the plan, including any benchmarks you have.
Dissemination Plan
Describe how you will share the results with others. Are the results relevant to others?
