Chapter ONE: Background
Unit Overview: 
Provide a general description of the microsystem including the
institution and the unit number. 
Include data from the 5 P assessment—particularly the Purpose,
Patients and Professionals (including unit leadership).  The information to be discussed is a
summary of that identified on page 6 (in-patient) or page 7 (ED) of the
Clinical Microsystem Greenbook.  The
actual Greenbook pages can be included in an Appendix at the end of the
paper.  This provides the context
for your proposed change.
Description of the Clinical Problem: 
Describe the problem/process/pattern for the focus of the capstone
project. Provide information on the Process and Patterns that impact the
process from your 5 P assessment.  
Describe how you identified the problem—meetings with leadership,
use of a process poster, survey etc, analysis of unit data, etc.  Describe how the problem impacts the
clinical and cost outcomes related to safety, effectiveness, timeliness,
efficiency, quality, or client-centeredness.  Why is THIS problem important to solve
in THIS setting at THIS time? Address how improving the process would help
benefit patients and what outcomes might be measured to evaluate the
project impact. A fishbone diagram is created based upon problem and unit
feedback.

Unit
Processes/ Patterns Assessment: Students are expected to gather input from unit staff about a
core unit process/ pattern on which they would like to focus. It is incumbent
upon the student to exhibit the leadership necessary to fully engage the stakeholders
in participating in this exercise. Evidence of completion of this assignment
will be provided by including a description of the feedback obtained. As an
appendix include any tool used to obtain stakeholder feedback.

TWO:  Literature Review
·      
The
literature review is an important aspect of project development.  The literature should help define the scope
of the problem, current nursing standards, and begin to develop an evidence
base for the project plan. The literature review supports the significance of
the problem. Look for similar problems/processes/patterns in the literature. Describe
how others have approached the problem/process/patterns and what has been
successful, how changes were measured. A minimum of 10 sources or more as
appropriate. You may find examples of projects with your specific topic, but
can also include articles about projects that are similar (i.e. different
safety problem) but use the QI methods similar to what you want to use to
address your problem.
·      
Briefly critique the evidence. A critique of
the literature should include:  strengths
or weaknesses of the research, and relevance to your setting and patient
population. Provide a conclusion of best practices to address your problem
based on the literature.

THREE:  Methods
·      
Chapter
three will include a detailed
description of the plan for implementation of the capstone project.   Include purpose and specific goals, and objectives for your
project. Describe the state of readiness of your setting. What are the barriers
or facilitators? What has been the reaction of the staff to you and your
project: Describe the key players in the implementation process. How will you
gain the support necessary to implement the project? Do you have ‘buy in’? Why
or why not? Describe the specific implementation strategies and a timeline to
provide a blueprint of how the project will be completed (who, what, when,
where, how).

FOUR:  Evaluation Plan

Describe
how you plan to evaluate the implementation of the project and success of the
project.  How does the plan impact
the problem you are addressing? What
data will you need to determine if your project was a success and how will you
obtain that data (I.e. clinical and/or cost outcomes related to
safety, effectiveness, timeliness, efficiency, quality, or client-centeredness)?
If you must develop an evaluation
tool, include that as an appendix to the paper. If you have pre-data, provide a
table and summary of this data and what changes you hope to see.

Role:  How does this project relate to the role of
the CNL? How did your education thus far prepare you to implement this
project?   What CNL role functions are
the most salient to this project? How does your perspective differ from that of
a staff nurse as you approach the problem/project?

FIVE:   Conclusion
·      
Implementation: Summarize how the project was implemented,
who was involved, the challenges that occurred, what went well, and any changes
in the plan that were needed.
·      
Outcomes: 
Describe results of your evaluation. Provide data on measurable outcomes
and include data tables. Also discuss any observations or feedback obtained
from nurses, physicians, leaders, or patients related to the change.
·      
Evaluation: Was your goal met? Identify factors that
contributed to your success, and barriers that may have existed.  What might have been done differently? What
recommendations would you make for future projects? What conclusions can be
drawn?
·      
Sustainability: Is sustainability recommended? Describe the
likelihood of project sustainability and contributing factors.  What conditions need to be in place for
sustainability of your project?
·      
CNL
role:  How did the project assist you in your
development of CNL role competency?  What
aspects of the CNL role were most relevant to this project? How would you
change your approach in the future if attempting to implement a similar
project?

Concluding
remarks
Final
paper must also include an abstract
·      
Write
an abstract of up to 300 words for the CNL Summit, per summit guidelines, and
include it at the beginning of the paper. The abstract should be written on a
separate page with a title and sections for each of the following:

Background
Information (problem and why important)
o  
Aim
(purpose of project)
o  
Methods/Programs/Practices
(what was done)
o  
Outcome
Data (what were changes resulting from project)
o  
Conclusion
(implications for practice, other CNL’s)

Problem: Lack of data to support if Skill lab videos prepare students and/or are useful for SLU traditional students within the clinical setting.
