Choose a specific chronic pediatric health condition of interest to you and one that poses potential barriers to learning in school-aged children.
Find 3 peer reviewed journal articles relevant to your topic. At least one article must be from a school nursing journal (Journal of School Nursing or NASN School Nurse). You can also look at pediatric, nursing, medical or educational journals for the other two journal requirements. Search for the most recent publications, preferably within the last 5 years.
Search the internet to find 2 websites with evidence-based practice information relevant to your topic. Websites that end in .org and .gov will provide the best evidence. Avoid .com websites as they may be profit driven or not have EBP information. The websites that you choose must be based upon sound and cited evidence.
Synthesize the information from the articles and websites to describe the evidence that you will implement in practice. Write one paragraph summarizing how the information from the articles and websites will inform your school nursing practice. Provide specific examples of the recommendations for the care of the student with a chronic health condition in school. For example, the student with diabetes should have a 504 plan, an emergency action plan, and an individualized healthcare plan, etc.

Copy and paste the table into a Word document. It will expand as you enter the information. Tables do not require full sentences. You may use phrases and bullets. Be concise.
Journal Information: Author, date, title of article, source (name of journal, Issue, page number).
