This assignment focuses on conducting a community health assessment for a chosen population. the assessment is focused on the social determinants of health and their relevance to the chosen community ( demographic: Calgary north east Alberta/ chosen population: hypertension).
Choose a community and provide an overview of the population that is supported by appropriate scholarly resources. Then, provide a rationale for the health assessment of that community that is also supported by appropriate scholarly resources. Choose four social determinants of health (SDOH) ( immigration, income, stress bodies and illness , unemployment and job security) and explain how they influence the health of this community.

Select 3 SDOH
Use Raphael et al. (2020) as your source from which to select the three relevant SDOH.
Demonstrate, clearly and thoroughly, how each SDOH impacts and influences your chosen community/population.
Provide a concise analysis of the assessment data provided, supported by relevant scholarly references.
Conclusion
Summarize clearly the main ideas in your paper.
Include thoughtful reflections.
Include recent, relevant research evidence using a minimum of 7–9 scholarly resources.
