2. Explain how the caritas processes and clinical reasoning cycle you        discussed in question 1 could be used to achieve solutions to address the care needs for the person in the scenario.
You are the nurse caring for Damien on the night shift. 
Damien Arnold, 18 years old, was brought in by his mum (Margaret) to the local regional hospital ED. Margaret is very worried because she said her  son never gets sick but lately he’s staying in his room and not eating. She said this is not normal for him. He is usually very active, playing multiple    kinds of sports and works at his dad’s electrical business. 
Margaret gives you the letter they got when they visited their local doctor three days ago. It says he sustained an abrasion on his left forearm. You  notice it appears cellulitic.  
Damien’s symptoms the past two days are feeling unwell, weakness, loss of appetite and thirst. Damien thought that was from all the alcohol he     drank after the footy game. He has no pain. 
His past history includes Wegener’s granulomatosis (a rare inflammatory disease that targets the arteries and veins in the kidneys and respiratory  system) and a kidney transplant three years ago. He is on                              immunosuppressant medications since the transplant.  
Damien is given a triage score of 2 (needs to be seen within 10 minutes)   and transferred to an acute bed. 
When you take over care of Damien at the start of the night shift you         notice he is pale, lying supine with his eyes closed. The nurse handing       over commenced the adult sepsis pathway. Margaret has gone to call     Damien’s dad but will return after that. You are assisting a junior doctor   with cannulation as Damien needs blood cultures. The doctor doesn’t use gloves when inserting the cannulas. You are worried because Damien is    immunosuppressed and septic.
