Explain your data collection plan to measure your intervention’
Office staff collected the pre- and post-weight during in-office visits and notated them in the participant’s medical record. Through a medical record audit, I collected and recorded the pre-and post-intervention participant weight on an Excel spreadsheet for comparison. The weight data is de-identified. There is no need to connect the weight data to each participant because the mean weight pre-intervention will be compared to the mean weight post-intervention. The de-identified data will be entered on the spreadsheet and saved to the password-protected computer. Pre- and post-intervention weights will be compared and analyzed to determine whether the intervention affects the weight.

A paired t-test will be utilized to assess the effect of the intervention on participant weight. the initial (pre-intervention) and post-intervention weights of the participants were collected through chart review, and the results were on an Excel spreadsheet for analysis. A decrease in participant weight will indicate that utilization of the National Institute for Kids Healthy Quality Obesity Toolkit for weight reduction effectively decreases weight among obese adolescents, thus a practical resource for providers in screening and educating identified obese patients.
