According to Healthy People 2030, Social determinants of health refer to the social and economic conditions in the environments where people live and work that affect their overall health outcomes and risks (U.S. Department of Health and Human Services, n.d.). Problem Description

While not related to pediatric care, a research article by De Lew and Sommers (2022) highlights the negative impact of missing data in federally funded healthcare programs on health outcomes, which worsens health disparities. The article emphasizes the importance of effectively measuring and tracking health outcomes to address these disparities. As the well-known saying goes, “We cannot fix what we do not measure” (De Lew & Sommers, 2022). Therefore, it is crucial to prioritize collecting accurate and complete data in healthcare programs to ensure marginalized communities receive the care they deserve.

The underreporting of social determinants of health in the pediatric care and preventative care for pediatric patients is a significant problem that adversely affects pediatric patients and their families. By addressing this issue, we can improve their quality of care and ensure they reach their full potential. Acknowledging the critical role of social determinants of health in healthcare service delivery will positively impact pediatric patients’ health outcomes.

Available knowledge

Currently, the clinic uses a dichotomous tool to verbally deliver the screening process over the phone before and in person at the beginning of the appointment. Based on the families’ responses, the clinic provides what they need during the scheduled mobile clinic. However, the issue arises when the families report additional needs in person not mentioned during the initial screening process.

Rationale

Using Peplau’s Theory of Interpersonal Relations and Normalization Process Theory facilitated the development of a revised Likert Social Determinant of Health screening tool to address families’ needs comprehensively while ensuring its efficacy. The aim was to ensure that the intervention is effective and does not overlook any families’ needs. The successful application of these two theories has resulted in an intervention that is expected to be highly effective while ensuring that families’ needs are not overlooked.

Peplau’s Theory of Interpersonal Relations was employed to establish therapeutic relationships between mobile clinic staff and pediatric patients’ families needing services. This approach enabled the team to engage with the patients more personally, thus building a foundation of trust that fosters better communication and cooperation.

The Normalization Process Theory was applied to streamline data and eliminate redundancy and inconsistencies in the intervention. The revised tool created through this approach enhances the efficiency of the intervention by simplifying data collection and analysis, enabling mobile clinic staff to provide better and more accurate services.
