Your survey should consist of 5 demographic questions and 10-15 questions that will help to answer your overarching research question. Make sure your research question is located on your survey.
Format: Your questions should be evenly spaced out. Remember to include responses for your participants: multiple choice, true/false, or Likert scale. If you are using a scale (eg Likert scale) the choices should be evenly spaced out as discussed in the chapters of either textbook. The survey should be on one page but can stretch to two pages, as long as you make sure the participants know there are two pages.
Keep in mind you will use Excel to analyze the data so close ended questions will be tallied and open ended questions will be compared to each other and evaluated for common themes that occur for each question. It is best for all questions to have available answers for the participants: Multiple Choice, Likert Scale, or True/Flase.
For this survey, participants will not identify his/herself by name, so please make sure you do not have a question for name. The questions that you ask will totally depend on your research question.
The survey should be easy to read: at least size 12 font, free of grammatical and spelling errors, and organized with numbered questions and/or bullet points.
