• Assignment 5: Completed Questionnaire Paper [rubric] – 100
Using page 55 in stage 2 of the Consulting with Nonprofits reading as a guide, you will submit the completed performance practice questionnaire, along with a 5+ page APA formatted paper stating what information was collected that will inform your project. The paper should include the following components.
• Introduction
o Describe the paper components, and this is a paper part of a larger consulting project.
• Methodology
o Explain the components of the questionnaire, the types of questions, and what is asked. Assume your reader hasn’t seen the questionnaire.
o Explain the data collection process you took to get the questionnaire completed. Describe how you emailed it to the organization and their response.
• Findings
o Summary of questionnaire responses. The organization will need to include some comments along with their scores. Send back the questionnaire if it doesn’t include comments.

• Implications
o Describe 3-5 items that arose from the questionnaire results that you will address in your implementation plan

Stage 3: Implementation
Now that the data has been collected, phase 3 involves making recommendations for the implementation plan. You will utilize many readings and resources you learned during your program and include them in your implementation plan. You will complete the presentation outline, the Implementation Plan paper, and the final presentation in this phase.
