Please find attached the template for concept map (system d/o template). This will be in lieu of a care plan. You may use the template as-is or ditch it and use a Word document with the appropriate headers (alterations in health, Patho, Risk factors, etc.). I’d prefer the latter, but again, up to you.
For Lab tests: it does not have to be a blood/serology lab test, it could be (and please/you MUST include) a risk assessment, pt questionnaire inventory, etc. Please ensure to include risk assessments you’d and/or inventories IN ADDITION to any serology tests that you’d request from the provider.
Meds: please list 3 medications (from different drug classes, if possible) that would be recommended for this diagnoses and 3 expected side effects (SE), and 3 adverse effects (AE) per each med. List the indication, any recommended therapeutic range (like for lithium), typical starting dose, and INDICATION. If the medication has a therapeutic level, please list.
Therapeutic Procedures: Please list three nursing interventions related to your at-risk diagnosis and expected outcomes please include a timeline of when you’d re-assess this patient.
Expected findings: what is the clinical picture supposed to look like? How is the patient most likely going to present? Describe thought-process, insight, and judgment, affect, orientation, etc.
Nursing goals/interventions/outcomes: Make the goals SMART-Specific, Measurable, Attainable, Relevant, Time-based.
**Not Included on Template, but Mandatory**: Please include 3 at-risk nursing dx for the psychiatric dx.
**Not Included on Template, but Mandatory**: Reference Sheet in APA format, referencing your citations AND include a cover page, too.
RE: Interprofessional care: who is going to do what in the hospital regarding taking care of the pt. This includes: MD, RN, LVN/LPT, social worker, discharge planner. Explain in detail.
RE: Patho: provide as much science as possible! Try PubMed, Google Scholar, or Mayoclinic.

PPS: you don’t need to worry about Erikson’s stages, drug use, etc. that is on the rubric not on this system d/o template or these additional requirements I want form you (above). I “exempt” those portions since that’s used for an actual patient.
