Expand upon the clinical practice area/problem that you described for EBP Table assignment. Present your findings from the literature and web-based resources on the significance of the problem globally, nationally, regionally, locally, and/or to a particular patient population or practice setting. Discuss the relevance of your identified problem to health care, nursing, advanced practice.
2) Problem Statement (5 pts) One paragraph.
Present the overall purpose/aims/objectives of the EBP project. Write a problem statement in PICO or PIO format.
3) Presentation of the Evidence (25 pts) Minimum of 2-3 pages (double-spaced). Remember to cite sources using APA format.
This section is a critical appraisal and summary of findings regarding the intervention/approach chosen to address the practice problem.
Describe the intervention/approach you have chosen to address the problem you have identified in the clinical practice setting (5 pts)
Present your rationale or justification for the intervention/approach selected. (5 pts)
Present a summary/synthesis of the evidence for your chosen intervention/approach to resolving/improving the practice problem. This section should not be presented as an annotated bibliography, with each paragraph presenting a discussion of a single article. Instead, cluster your evidence into units of meaning (paragraphs covering the types of evidence or outcomes that support the intervention) and if more than one article provides similar evidence, paraphrase the overall findings and cite the multiple sources in a single citation per APA format. (15 pts)
4) EBP Model Chosen to Guide the EBP Project (20 pts) Approximately 2-3 pages (double-spaced). Remember to cite sources using APA format.
Description and discussion of EBP model chosen to guide your EBP project. Describe the implementation process that you would use to address the problem. Discuss the feasibility of making a change in your own practice arena. Present your recommendations for an organizational change theory that would apply to your project and provide a rationale for how the change theory integrates with the EBP model. (20 pts)
5) Anticipated Outcomes of the EBP Project (10 pts) Approximately 1-2 pages (double-spaced).
Present your recommendations for evaluation measures with rationale. Discuss what kind of outcome data you will need to collect for evaluating the project. What data will you need to collect, that is, what will you need to measure/assess in order to demonstrate support for your evidence-based intervention? Consider what measures were used in the studies you have reviewed that provide evidence for the effectiveness of the intervention chosen to address the practice problem. Consider what data supported that the problem was significant and relevant; that is, data that you might expect to see change after you implement the intervention.
5) APA Format and Style (10 pts)
Reference list, citation format, scholarly writing style, paper in APA format.
