What do you think the pros and cons are when a formerly prescribed medication becomes available OTC?
As a prescriber, I find that insurance coverage can be the problem. For example Fluticasone nasal spray (known as Flonase) became available OTC several years ago. While it is great that the medication became more widely available, the OTC cost is not benign. The brand bottle was over 12-15 dollars per vial, which can be costly to those with low income. Additionally, there are time this medication needs to be prescribed in order to treat with different dosing that on the package. In this event, a prior authorization is then needed and can delay care.

In order for a drug to get FDA approval as an “over-the-counter drug” it must have low potential for misuse and abuse, can be used for self-diagnosed conditions, can be used safely and effectively without a health care provider’s instruction and can be adequately labeled. It is a pretty lengthy process of evaluation, that I think has several advantages and disadvantages. A very positive aspect of a drug becoming on “OTC” is that is become more accessible and affordable. I think that the major flaws in this process is that it is overly lengthy and bureaucratic. Sometimes drug companies can be incentivized to not make a drug an “OTC” because they are profiting off of it. Also it seems that some drugs that have become OTC’s are drugs with a large potential for abuse and dangerous side effects. For instance DMX and Codeine. I think that this process should be more flexible and be independent from drug companies.
