1. Indicate the specific type of HCO you are leading (e.g., Regional Hospital, SNF).
2. Indicate the casemix of the HCO, and the process of care (e.g., 1.5 and explain the number, and how the process of care is ethically processed).
3. Indicate the structure of the HCO along with all funding sources.
4. Indicate the quality of care standards (e.g., morbidity) of the HCO, and the mission and vision that guide your leadership. The Mission and Vision must be connected to your answer in part 1 with the Mission at least four sentences, and the Vision no more than two sentences.
