The Perez Family
You are a middle school nurse practicing in Toms River, NJ. Celia is an 8th
grader in your
school who is newly diagnosed with Type I diabetes. She has been on homebound instruction
for 6 weeks and will be returning to school in early June, just three weeks before her 8th
grade
graduation. The Perez family lives in a two bedroom apartment in Winteringham Village, a
large development on Rt. 166. The family consists of Mr. Perez (38), Mrs. Perez (36), their three
children – Celia (14), Alex (10), and Elena (3). Mrs. Perez’s mother, Yesenia (75) recently
arrived from Mexico and is now living in the home. Mr. Perez has two part time jobs – his
primary job is in a fast food restaurant and he works Saturday & Sunday early mornings at the
car wash. Mrs. Perez works 3pm-8pm weekdays for a commercial cleaning service. Yesenia
and Celia babysit Alex and Elena until Mr. Perez gets home at 7pm. Yesenia makes traditional
Mexican meals for the family, but most nights, the children prefer the fast food that Mr. Perez
brings home from work.
Celia was an excellent student prior to her diagnosis, but despite homebound instruction, she has
been struggling to catch up with her work. She would like to be a cheerleader, but her home
responsibilities prevent her from participating in after school activities. Celia takes the school
bus home, but there is no transportation available after cheer practice. Her parents do not want
her to be involved in any physical activities because of her new diagnosis.
1. Identify the factors in the Perez family case relevant to each of the five social determinant
areas (refer to Healthy People 2020 – SDOH):
● Economic Stability
● Education
● Social and Community Context
● Health and Health Care
● Neighborhood and Built Environment
2. As a school nurse, identify available community resources IN YOUR OWN TOWN
AND/OR COUNTY that would be appropriate for the Perez family.
3. Create an IHP for Celia.
4. Create a transition plan for Celia which addresses both her return to school and her
transition to high school.
As a school nurse, identify available community resources IN YOUR OWN TOWN AND/OR COUNTY that would be appropriate for the Perez family.
