1. I am in a psychiatric outpatient setting this semester where my preceptor treats acute and chronic psychiatric disorders. I will be able to accomplish my set forth goals by the end of this course. I will incorporate the use of evidence-based practice each clinical day with appropriate an up to date for the best approach to clinical data, I will aim to do this 80% of the time if not 100% while completing my outpatient child and family clinical experience at Bridgeport behavioral health clinic. With the Lord’s guiding hand and my preceptor at my side, I will foster good practice in using evaluation tools created by those who came before in psychiatry like GAD-7, PHQ-9, Cross cutter questionnaires, mood questionnaires, RCADS, TESI-C, MFQ, POSIT, PSC, HEADSS and Child PTSD symptom scales etc. I believe relationship building is very important in creating trust, which starts with the parents it helps foster a good working relationship in return it will assist me in building good family and medical histories, as well as completing a foundation of any mental illnesses that may run in the family. I will see at least 6 outpatient child and adolescent through the ages of 5 to 17 in every eight-hour practicum experience. As the bible foretells, “for I know the plans I have for you, declares the lord, plans for welfare and not evil, to give you a future and a hope” (Jeremiah 29:11).
I will perform laboratory assessments when necessary 100% of the time on medication assays like Depakote and lithium level as well as checking hepatic function, CBC, CMP, lipids and vitamin D, I will listen to the heart for murmurs and review all pertinent ROS. I will complete AIMS on my patient’s who are on anti-psychotic and monitor for EPS.I will implement the very heart of nursing in all that I do with compassion. I will educate on health promotions, infection prevention, psycho-social with noted limitations, and provide educational information on all medications I prescribe 100% of the time I will assess the knowledge of my patients and family members to ensure everyone is comfortable with the plan of treatment.
I will refer my patients to therapy 100% of the time to ensure proper coping skills are obtainable when necessary . I will assess each patient’s level of wellness, height, weight, vital signs and I will implement a better understanding of behavioral health financial matters with collaboration of my preceptor I viol to only prescribe medication my patients can afford. In addition, I will improve on performing HPI’s on new patient interviews at least 80/100 with special attention paid to new patients each clinical day and effectively complete half or all of documentation through the use of SOAP format for follow ups. I will be able to apply standards of evidence- based practices to set the foundation for examinations and satisfactory administration to my best abilities at least more than all the time. I will use up to date to ensure recommendations of pharmacological and non-pharmacological treatments are current.
